We can provide Functional Capacity Assessments and prepare Functional Reports for adult clients (please note these reports will be centered around home modification and equipment prescription and how much support needed in the home and community).

This service is currently for National Disability Insurance Scheme (NDIS) self and plan managed adult participants, Home Care Package (HCP), Short Term Restorative Care (STRC) and Commonwealth Home Support Programme (CHSP) clients.
